The Coordinated School Health - Survey
This survey is being conducted by the School City of Hammond’s Coordinated School Health Committee to identify the current knowledge of the available health and wellness services, and to determine what areas of interest need further attention. Once you begin the survey it must be completed in one setting, but should not take more than 10 minutes. Your answers are completely anonymous.
